AI Note Taking Consent

I understand that:

i. AI note-taking may involve audio recording and/or automated transcription of consultations to assist veterinary staff in maintaining accurate medical records.

ii. These recordings and transcriptions may contain personal information and information relating to my animal(s).

iii. All information collected will be handled in accordance with applicable privacy laws and the clinic’s privacy policy.

iv. Recordings and transcripts will be securely stored and accessed only by authorised personnel for clinical, administrative, and record-keeping purposes.

v. AI-generated notes will be reviewed by veterinary staff but may not be entirely error-free.

I acknowledge that:

i. I have had the opportunity to ask questions about the use of AI note-taking and recording.

ii. I may withdraw my consent at any time by notifying the clinic, understanding that this will not affect the care provided to my animal(s), but may limit the use of AI-assisted documentation.

iii. My consent applies to all future consultations unless withdrawn in writing.
