Surrender Agreement - FELINE

I clarify that I am the legal owner of the cat described above and that I have the authority to surrender this animal. I understand that by signing this form, I relinquish all rights and ownership of the cat to Quality Vet Care Centre. I understand that the organisation may evaluate, treat, or rehome the cat as they see fit.
